5yr old constant runs, when to go to docs?

My son is 5 and on Monday had a temp so kept him off school, was drinking and eating small amounts, so gave nurofen.

Tuesday, constant runs, mainly brown water and stomach cramps, bum was red raw so nipped out last night and bought nappy cream, he was at the toilet at least 20 times (lost count). Ate and drank small amounts.

Today, he was up at 7am, passed some more brown water and I've managed to get him back to sleep (still sleeping), he had a drink before going back.

Should I call the doctors? Im worried he will get dehydrated, he's eating so little he's usually very active and slim, now I can see his ribs .

Usually dehydration in children is exhibited by dry mouth, sunken eyes, and maybe lack of tears. A five year old should have the capacity to tolerate diarrhoea for 24 hours so if he still isn't vomiting then keep giving him sips of water or flat cola/lemonade. Food could be bananas & toast

I'd be more worried to call the doctor if he's still having diarrhoea after 72 hours, or starts vomiting.

Thanks, hes currently awake and having some toast and a drink, he did gag this morning at 7am when up for a poo. He hasn't been to the loo since being up, so hopefully thats a good sign, will see how quickly his toast remains inside his body!
Have been giving him flat cola, he thinks its a great treat.
