What's this rash?

DS is 16 months is generally a rashy/eczemaey baby, but this rash has come up over the last 48 hours and I'm stumped if I know what it is. They're small spots on his back, some of which are white-heads. I suppose a confirmation that it's not chicken pox or measles would be great!

If anyone can identify the huge open sore, then I'd also be grateful, but having seen GPs and going to see a dermatologist next month, don't worry if you can't!

some kind of heat rash? does he have waterproof bedding? that can make them really sweaty (thinking if he sleeps on his back that might have made it come up there?)

viral rash?

could the sore be a patch of infected excema?

He spends all his time on his front (sleeping etc), and when he is on his back involuntarily (nappy changing etc) he complains like mad, wriggling and all sorts.

We've used antibac cream, antifungal cream, emollients, hydrocortisone etc on the large scab with no luck.
