Swollen hand - should I be worried?

My DD is 19 weeks and is a thumb-sucker. Her hands are always cold and they do have a blue-ish tinge sometimes, but the 'pad' of the thumb on her left hand is quite swollen. Its not painful to her, and she still continues to suck that thumb. Should I worry?

if concerned with a littlie its prob always best to get proper medical advice but fwiw, even as an adult I can see the diff between my sucked thumb and non sucked thumb (sucked til I was 9 )

2nd what ohyoubadkitten says, but could it be a blister from sucking too hard?
