question on 8 months poo

my 8 month old dd is having problems passing stools. they are really solid and causing pain.
i am going to get graphic now but she was trying to poo yesterday and started screaming. when i took her nappy off she had a massive poo hanging out. it was about the diamensions of a 10p peice. if was really painful for her.
my question is what can i do to soften them

does she eat plenty of fruit and veg?
Maybe some dilute fruit juice can help?
If its really hurting her take her to the docs tomorrow, he might be able to prescribe a gentle laxative, like lactulose.

she eats loads of fruit and veg. have tried to give a few spoons of fresh orange but does not seam to be helping
