does it normally stick out like that?

can't remember what dd's looked like but it was an obvious lump rather than just skin iyswim (though it wasn't big)

I think tummy button hernias only happen to babies, and would prob look bigger than what yu describe.

Could he have been picking at it, and maybe rearranged the skin folding iykwim?

It would be more of a large bulge than a blob of skin.

DBro had one as a baby. He had it operated on and now has no tummy button at all. He likes to tell folk that he hatched.

I think umbilical hernias can happen at any age, but it would be a fully-fledged bulge from within, IYSWIM.
