Splinter removal, v swollen foot

DS5 was barefoot in the garden a couple of days ago and trod on something - not sure what but it was very painful. DH removed what he thinks was a bit of a splinter. DS's toes and foot are now extremely swollen, red and painful. We went to out of hours nurse practioner this morning and she said it could be an insect bite or a splinter but she couldn't see anything in his foot. She gave us antibiotics and said to also take antihistamines. I've since taken a couple of zoomed in photos and think I can see a very small sliver of the splinter (or something?) still in there. Wondering what the best thing to do is...leave it? Try and get it out? Any advice welcome!

If you can see something, try to get it out maybe? Soak his foot and use a calpol syringe?

I'd have him soak it in Dettol/Savlon probably. If it looks near the surface maybe push down on the skin either side or pull with some fine tweezers. If not, hydrocolloid plasters might work to pull it to the surface.

It's tricky as I can only see it in photos and not with the naked eye. Will give it a soak and hopefully that will draw it out.
