DS struggling to poo

My son awkwardly admitted to me today that he has been having trouble doing a poo and will go usually once every 3-4 days for about two months now. I always make sure to give him enough proteins and fibre in his diet, and he doesn’t snack much, bmi is healthy etc. He does have a phobia of public toilets and will not use any of them either out and about or at school, which is a problem in itself and one we’ve been working to overcome, so I do wonder if this is related? He’s 12

When at home yeah, I got him a stanley cup and make sure he drinks plenty, because when he’s at school he avoids drinking as to not need to wee
