8YO withholds poo

I feel at the end of my tether. DD is 8 and does whatever she can to avoid pooing. She's always squirming, like she has ants in her pants.
She's under a paediatrician who has said to reduce the movicol that she's been on for 4 years and to get her to drink more water.
Since doing this everything has got worse. She now has poo in her pants everyday, I don't know if it's overflow poo or from withholding.
She refuses to drink water / squash if I ask.
It all feels like a huge battle of wills that the doctor doesn't seem to understand.

Anyone been through similar?
