Am I worrying about nothing?

So im hoping someone can help in some way, advice maybe? Opinion?
My son is 4.5 year old, he's always been a very active, loud, confident little boy but for the past year me and dh have noticed things that are worrying us.
He's a very emotional little boy, he will cry if its sunny and not raining for example.. if water touches his ears, if he drops a piece of food and so on...
He's a very outspoken boy, he literally has no filter at all and even when we've explained that something he said could be hurtful he really don't seem to understand, just tells us he don't care or it turns into him pushing us away (physically)
He's very fidgety all the time, could be watching TV but he will be in 50 different positions
Walking down the street holding our hand and he will be jumping skipping hopping, our arms get wripped out of socket!
If he falls or does anything that is embarrassing for him he will attack us
He will say mean things to people and again, not understanding why they're upset (other children)
He has developed what we believe are vocal tics, (sounds like throat clearing) been doctors before who said "postnasal" however they are still there now and only seem to appear when he's overwhelmed which is normally due to a change, his dad suffers with tics
His sleep isn't always great either, he has a lot of night terrors
He still doesn't like to play alone, he will even follow me and dh to the toilet, we can't even talk because he talks over us and gets louder and louder and louder before jumping all over us to stop us talking...
Honestly we have tried everything, even went through behavioral support.
He's around no negative behaviour, no sudden changes to his life or anything like that.
Am I wrong in thinking this isn't normal behavior?
I'm so upset because I don't know what else to think or do to help him
He is my only child so I am probably worrying over nothing?
