1 year old suffers when pooping

So basically my son is now 14 months, for a very long time now he just struggles so much when he has to poop, he will poop once a week, when it's coming he will kick his legs like he's trying to hold it, this can be for a few days before it just has to come out and he really really panics when it's coming out. Sometimes I have to use windi (I'm writing this post after using it at 2am because he woke up and couldn't get back to sleep in his pain. Even when using windi it starts to come out but stops like he doesn't know to push or is trying to stop it somehow? We just have to try position him to push it all out then he is fine. I don't know how to fix it! We give him water, oranges, pears, he's not constipated as it's a normal soft sausage poop (sorry for tmi lol), all the doctor told us is to give more water. He's exclusively breastfed btw and has solids like egg or oats, fruit and yoghurt, rice and veggies. He really really panics when pooping and I don't know how to help it or when it will get better.
