Hi just wanting some advice my health vistor rang me the other day as she is leaving and I will be getting a new health vistor so she wanted to tell me all the information she thought I should I know, she informed that I could claim dla for my 18 month old girl she hasn't been diagnosed with anything but has suspected asd and also suspected hip dyslasia, but awaiting appointments with a pediatrician, physiotherapy, speech and language and dietician which all have waiting lists I have the forms to claim dla but I'm just stuck on how to fill it in as she hasn't been seen by anyone yet just health vistor should I wait until she has been seen by everyone she has development delays without a doubt this is what the health vistor is telling me to claim dla for as she needs more care than a normal 18 month old would ?

You don’t need to wait. Use the Cerebra guide when completing the form. Ask the HV to write a supporting statement/letter before she leaves, too.

She has already left would a new health vistor be informed of everything would she be able to write a statement?

The new HV will have access to the notes and should be able to write a letter, but I would speak to them first to make sure they are up to speed.

You could request copies of the referrals too.

You can submit a subject access request to get them.
