15 week old clammy

Hi, my 15 week old DS has been very sleepy and fussy for a few days and today his skin is clammy to the touch. After his nap his clothes are damp where he has been laying. He doesn't have a temprature and is cool to the touch.

When he is awake he is alert. He also goes from smiling and happy to screaming and tears within seconds.
Should I be taking him to the doctors and is there any advice.
Any help appreciated.

My little one is the exact same, where is he sleeping? I found it my dd contact naps or cuddles into something soft and cuddly then she becomes warm and sweaty so I let her fall into a deep sleep then move her so she’s not got too many layers on.
