Nigh time retainers query

Following orthodontic treatment DC had a night time retainer that they were told must be used for the rest of their life. It has wear and tear and does not look clean in spite of using the tablets for cleaning. Who to contact to get a replacement?

When the orthodontic treatment was finished they said nothing to do with them anymore, but surely and ordinary dentist (if ever manage to get an appointment!) can sort this and get a replacement? Wouldn't the orthodontist hold on to the moulds or are new ones made every time a replacement is needed.

And what happens after they are 18? So difficult to find a NHS dentist or get an appointment!

Looking at other braces and retainers threads I can see some mention retainer checks - this was never mentioned by the orthodontist or the receptionist so my teenager has not had one. Ever.
