Possible ear infection

My 3 year old started with a high temp and vomiting early hours on Thursday morning. I ended up phoning 111 after he threw up the Calpol I gave and he kept being sick every 10-15 mins. We were booked in to out of hours to be seen at 5am. His temp was 39.7 and Dr gave Ibuprofen, said temp needs to come down and if continued being sick for 2 hours take to A&E. (Give 5ml water every 5 mins.)

I ended up taking him back to GP at 4pm as although he wasnt continuously being sick, I could not get his temp down. He was sick after Calpol and only kept Ibuprofen down but it didn't help his temp.
Dr prescribed paracetamol suppositories, his temp came down.

His temp has been up and down since then, it does come down with Calpol or Ibuprofen. He is complaining of sore ears, Saturday was the worst day but he does seem to be picking up. (Eating small amounts now, temp comes down with meds, has about 3-4 hours of feeling ok before crying about ears and temp rising. )

I don't know whether to take to the Dr again or if we just need to ride it out. It's so distressing when he is in pain, I dont know how long I should be waiting to see if he just gets better. I hate giving him Calpol and Ibuprofen all of the time but he is so distressed when his ears hurt and his temp is around 38.1 - 38.5 when it rises.

Thoughts please.

If his ears are sore phone again.

Poor DS ended up in a terrible state with an ear infection when small & needed the strongest antibiotics to sort it.

Sounds like antibiotics needed
