Getting baby to take antibiotics

DD is 5 months and on a long term oral antibiotic awaiting an op. She's taken this well so far syringed into her cheek but has firmly turned against it, and now gags and vomits bringing her feeds and meds up.

Tried/considered:
Different time of day, though she's meant to have it at night
Doesn't take a bottle/dummy so can't try it in a teat
Went on nursing strike when I tried it on a nipple like vitamin D drops
Not weaned yet and assume it'll take a while before they reliably eat enough to mix it in food

Anything I'm missing? She's dropped centiles from being ill so bringing up feeds is stressing me out, plus the worry she's not getting the medicine she needs Sad
