Left testicle pain in my 13 year old

Hi just wondering if anyone had advice on testicle torsion?

Pain started this afternoon approx 1500 when walking up some steps, started as a stitch type pain in lower left abdomen. This evening he has developed pain and tenderness in his left testicle. slightly hurts to pee, nothing else unusual. I've given him 400 of Ibuprofen and we'll see how it goes.. just wondering on your thoughts of how quickly this could progress if it is a torsion, or whether it's an infection. We'll obviously see how things go overnight and if he's still got it in the morning i'll take him to the GP. Just want to be prepared to look out for any other warning signs.. Thanks

Phone 111 right now - they may well send you straight to A&E. Possible testicular torsion is a medical emergency, not a "wait till tomorrow to see how it goes" thing.

It's not severe though, it's a bit tender to touch. Doesn't hurt unless he sits down on it. Bit overkill to ring 111 just yet, obviously i'm going to if it gets worse. It could be a strain or a bit of an infection. He's quite happily playing his Xbox at the moment, just looking for worsening advice really

As the other posters have said. This is a serious emergency. Go to A&E and you should be seen ASAP by a dr. Take a bag of things as they don’t mess about when possibly a testicular torsion and may well take him to theatre for exploration of the testicle under anaesthetic as that’s often the only way to know for sure. Coming on here to third the above posts. Do not wait until the morning. Someone needs to see him tonight. A torsion is probably the most likely diagnosis and it is a medical emergency, not something to leave overnight. I would suggest A&E rather than 111.

Please don't wait for 111 - get him to A&E now... if this is torsion you have a very narrow period of time for them to save the testicle and he'd need to be in theatre within hours.

They won't think your making a fuss if you say your worried it's a torsion

Get to hospital right now this moment. Not in the morning, not tomorrow. No messing around with 111. If this is a torsion, then the longer it is left, the more likely he is to lose his testicle.

It could be a strain or a bit of an infection. It could be but first and foremost he needs to be assessed to see whether he has a torsion which is the more likely diagnosis. This CANNOT WAIT OVERNIGHT, do not wait to see if it gets worse, you need to get someone to see him tonight. If it is a torsion and you leave it overnight he may lose his testicle.

DH had a torsion as a child. The pain started at school and his male teacher rang MIL and advised an immediate trip to A&E. He was in surgery an hour later.

Well I'm only thinking Torsion as a worst case scenario, he hasn't been in enough discomfort yet to warrant that and i've examined him, there doesn't seem to be anything abnormal. There's no swelling, no difference in size. It's just a bit tender to touch. I'm trying to use some common sense here. I don't necessarily need to whisk him off to A&E right now that would be a bit dramatic. Obviously if it gets worse I will take him, Just wondering if anyone else had experienced it and how their symptoms developed really. But thanks for the replies so far

DS (11) had similar symptoms as you described (including mild pain). We debated about what to do, but after realizing there was a chance it could be a torsion, took him to A&E. So glad we did as it was and he was operated on within 90 minutes of being seen.
I understand that there is a risk that the teste could die if it’s twisted and left without any surgery.

My son had a testicle torsion when he was 15, he woke me up around 3.15 am, he knew something was wrong. By around 8.30 am he was having emergency surgery. I rang 111 when he woke up, they spoke to both of us and told us to go straight to A & E.
He really should be seen asap, the testicle starts to twist, the more times it twists the greater the chance of actually losing it. My son was lucky and didn't loose his testicle, although me and my DH went through a few hours of hell, when the consultant told us it could be cancer. It wasn't and my son is fine, took a good few weeks to recover though.

You really need to get him checked out asap.

Yes you do need to whisk him to A&E right now. Better to waste a bit of time now than for him to lose his testicle. By the morning, if it's a torsion, his testicle will be dead. Do you really think it's worth taking a chance?

This is from the Torsion Commissioning Guidance (funding):

"Diagnosis of testicular torsion cannot be reliably excluded by location of pain, imaging, positive urine dip for infection, or presence of pyrexia. In many cases it is not possible to determine the cause of an acute scrotum without exploration based on history and physical
examination alone."

This isn't something to gamble with.

He needs to go to A&E tonight. I can’t stress this enough.

I have seen several boys/men with torsion professionally and it isn’t always that painful. Sometimes it is “just a bit tender to touch” with no swelling or difference in size.

I'm trying to use some common sense here. in which case please please listen to the advice everyone on this thread has given you which is to get him seen tonight.

Thanks for the advice, I will monitor it hour by hour, if it gets worse I will get him seen asap. At the moment it's about a 2/10 pain so we will see how the evening progresses, thanks for the advice everyone

As the mother of a one testicled teen A&E ASAP! My son lost his as a baby and it's been drummed into us to go straight to a&e if any pain as there is a time limit on saving a twisted testicle.

My son had one. I think I was told there was a 6 hour window before it can cause irreparable damage, this is a be on the safe side situation.

I know a boy this happened to and he lost his testicle and now has reduced fertility.
Stop wasting time and take him to a and e. Angry

There was no swelling in my son's case, it was just tender. Even if it turns out not to be a torsion, you really need to get him checked out. If you leave a torsion too long, the testicle will need to be removed. It's not worth taking that chance.
