First time mom here long time stress head

My little boy was 3 weeks early .. He had a fistula when he was born basically bit like a hernia part of his bowel was on the outside and he had surgery at bch . Discharged after 2 weeks no issue

We really struggle to wind him ...He throws his head around alot when you try and it's definetly hit and miss if we get a burp

Some nights not all he can be massively unsettled ....He doesn't really cry thank god but he will lie in his cot grunting and groaning and flailing his arms for hours on end . He appears distressed but he is asleep at the time ... if we pick him up he will calm down but back down he goes after next feed and he starts again

As he doesn't cry I don't think it's colic ....can you have colic but not cry about it ?

Reflux maybe . He does throw up some milk some feeds but not on a regular basis ...doesnt bring legs up to tummy or anything .

I did find something called grunting baby syndrome but the arm waving and noise last night was so next level I am doubting that too now

This doesn't happen every night which is even more confusing ...the night before last we went down calmly all night no issues no noise.

Before I tried to contact the hv wondered if anyone else had experienced something similar .. my gp isn't doing appointments right now :(
