Any ideas what this is?

This appears every so often, usually when my son has cried but not always. It is a raised white spot, almost like a bite, surrounded by a red patch.
It’s in exactly the same place every time and then eventually disappears.
He is almost 14months and I’d say I noticed it a couple of months ago. At first I thought it was a bite.

A vein?

Do veins go white with the red circular patch around them? I’ve never seen that happen before 🤔
