Reoccurring perianal/rectum rash

My son is 4 and has had a rash that looks like the perianal strep rash come and go since he was 2. He got scarlet fever, lymph nodes swelled and since then we have this ongoing rash problem. He has had blood tests, Food allergy tests, strep tests always come back negative and intestinal parasite stool tests and he has been on three different antibiotics because the doctors last guess was chronic strep carriage. He has seen Dermo, immunology, and gastro at a Pittsburgh children’s hospital. Still no answers. Only trend is he gets tired for about a week, super emotional and cranky for a couple days then the rash appears. I believe he has also been tested for Epstein Barr. The rash can have a defined ring or is just very red and raw in a circular shape around the anus and is painful. Any ideas or experience? We are reaching out on forums now bc he keeps stumping specialists.

Following due to my boy same age has the same thing you described I’ve been curious as what’s causing it
