Nappy/heat rash or thrush? Or something else?!

My son is 6, although the size of an 8 year old. He is incontinent and non verbal and wears nappies full time (Tenna slip). Anyway the past few days he has had very red skin in the nappy area, which I assumed was just nappy rash which he gets a lot and can be triggered by certain foods (waiting to be seen by allergy clinic).

Anyway on further inspection today as it looked worse, it's a lot worse than usual and has white, large lumps, surrounded by bright red angry skin. It's kind of under his testicles but to one side. It is then only that side of him that is red and angry looking, the other side looks normal.

Could it be thrush? Do I just get an anti fungal cream or do I need to take him to get checked? (He won't be happy being checked and has hurt me just by me checking).

Or might just be the heat and the weather? It is bothering him.

He clearly needs some nappy free time but not sure how to Do this without spreading poo and we're everywhere. He has frequent runny stools and very little understanding.

I would definitely try and take a photo and show a pharmacist or GP (explain the issue with checking). If not they can often recommend a cream to try just from the description.

Can you tie a muslin/tea towel loosely round his hips in a nappy shape to try and contain as much as possible? Or let him run free outside as you can just hose down the outside if needs be.
