Discomfort in the private area

I'm wondering if anyone can help, since picking up my DS from nursery this eve he's been very unhappy and distressed, we checked his nappy (as we could smell he needed a change) and there was a small poo in there (his nursery diary said he was last changed with a soiled nappy at 3.40) this was now 6.30, started to clean him, and he became very upset and seemed to be in pain with my cleaning him, it was a little dried on around his private area and his testis were really quite red, and he became more and more distressed, I put some cream on the area and tried to calm him, but I can tell he is in pain, could this just be from the discomfort of the poo being in there to long and causing a rash? Or could there be something worse at play here? It is now 10.50pm and he has woken 3 times screaming and crying, and when he is asleep he is whimpering, the testis don't look swollen, just red and tender when touched. Any advice would be great, if he continues to show discomfort in the morning I will of course be taking him to the doctors!
