18 month old screaming and not eating

DS has spent the majority of the night screaming uncontrollably. Completely out of character for him, usually sleeps all night, rarely cries and easily consoled when he does. I managed to get him to fall asleep on me a couple of times so he managed a couple of short stints of sleep but woke again very early this morning screaming.

He hardly ate anything yesterday but was otherwise happy and fairly normal. Went to soft play in the morning, visited grandparents, pretty average Sunday. He won't eat or drink anything today. Wet nappies as normal, did a poo yesterday morning.

I've managed to calm him down now by driving him round in the car and he's now watching telly fairly happily but still won't eat or drink anything. The only unusual thing I can report now is his breathing seems a bit strange, on his outwards breaths he makes a grunting noise, a bit like his trying to do a poo.

Doctors or just a quiet day at home do you think?
