Poo accidents at nearly 5

Hi all,
Before I start, I'll just say that I have 4 older kids, and I know that toilet stuff tends to come right and it's not worth panicking over.
That said, my almost 5 year old and soon to be at school has always struggled with a vicious cycle with pooing. He will get terriblly constipated then I go to the Dr's and they give me cosmocol then he gets too runny, I control it for a bit then it goes one way or another.
For the last 6 months tho he's pretty much pooing accidents all the time. It seems like sometimes he doesn't know, other times he doesn't want to.
We've been to the doctors, health visitor, and consultant (he has other stuff, weak core, turned in legs etc) and no one has done anything to help. We're on a waiting list for a clinic, but the NHS seems totally broken at the moment and they've admitted it could be months.
Really, I'm so worried about him going to school. The teachers say they'll sort little bits out, but it can be an almighty accident at least once or twice a week. He's very social and i know it embarrasses him.
