22 month old pain?

Hi everyone. I have a 22 month old girl and in the last month she has been pulling at her nappy and saying "wee wee" and sometimes crying. I thought initially that she was ready to be trained but after a few tries it hasn't worked at all. Then I thought she just hated nappies but when I would take it off she would scream the house down. 2 weeks ago I brought her to the doc and she did a swob and urine test and all came back negative. This week it is worse and she won't go to bed by herself and keeps tugging and pulling at her nappy. She also feels her back at times and when I rub it it settles her. She is going to the toilet fine and no temp etc. Plan on bringing her to the doc again this week but anyone else have a similar experience? Thanks.

Could she be constipated?

Is it worth changing her nappies? Maybe they are rubbing against her? Or maybe she has trapped wind?

My dd is only 8 months but has suspected kidney reflux (vesicoureteral reflux if you want to Google) she has an ultrasound in two weeks. I had the same when I was little bit I wasnt diagnosed until I was nearly 2, my mum says she just thought I was a screamy baby!
For your information though, if the doctor sent the urine sample away and it was clear then it's unlikely it's that, but if he just dipped it with those dip tests then they don't show anything, I was told the first time it showed white cells but no nitrites, however when it was sent away as a precaution it showed infection. We now take a weekly sample for analysis until she's had the ultrasound as the wait is ridiculous here!

Thanks guys. Tried changing the nappies but no use there. The urine was sent away that time too and came back clear. Hopefully doc will no more. I was thinking pin worms but I don't know. Hopefully nothing 🤔
