Help - blood in toddler's nappy

DS is nearly 3, has been fine all day, latest dirty nappy had a spot of bright red blood in it, but it was up at the side somehow? Like where it would rub his groin. He had sudden bright red nappy rash as well with skin a bit puffy in places but not broken so I don't think the blood is from broken skin.

Kicking myself I didn't check his nappy earlier as he was in it for quite a while but he's already had a dirty nappy earlier today and when asked insists he didn't have a poopoo in his nappy (we are trying to encourage potty use so haven't been 'pushing' the issue).

As said, he's fine, charging around although didn't like us wiping the sore skin. Any ideas? does he need to see the doctor or should I wait to see if it happens again? Have put metanium on rash which usually clears it up in no time.
