10 week old constipated?

my 10 week old went through a week of constantly pooping but it was quite soft and almost wet which was happening about 3-4 times a day for 2 days so I took him to the GP and they said it was a viral infection which should pass in the next few days.
He has Reflux so we put him on Aptamil Reflux milk about 2 weeks ago, he was fine for the past two weeks and it helped so much he's never ill after feeds anymore and takes them so much nicer than normal non-thick milk.

However, yesterday he didn't poop at all a few times I thought he was trying too and wanted too but nothing but this morning about 3am he woke up and was straining very hard to the point I thought he was hurt, I left him to finish his business for a minute until he started crying. I took to change him and in his nappy was a hard rock shaped poo which I then realised why it was so hard for him to do. The poor thing was in pain and around his anus was quite red. After passing that rock he did a 'normal' soft baby poo. The colour was yellowy-green both ways.

I've put some nappy cream the GP suggested on and he's yet to do another one (which is normal since it's only 7am right now) but I'm worried I hate seeing him in any pain. Is this normal every once in a while?
