ASD "diagnosis" tool?

Sorry really couldn't think how to word he title!

I'm sure I saw a link on here once to a page with tick boxes which then gave your child's "risk" of ASD (for want of a better term!) does anyone know what I'm talking about or have a link?

AndNowItsSeven the one posted is for 4yo as well, I use it for my 4yo, and it's meant to show the progress if any from early intervention
