almost black gritty bits in nappy (child)

Ds (nearly 6) is disabled, he is in nappies. He has constipation, and is on movicol. Recently I have noticed that often there are times when his nappy has a smear of very dark poo, almost black, and bits of what look like old gritty poo, it is making him quite sore.

Any idea what could be causing this? It doesn't look like his normal poo, and not like the shmearing he gets when he is constipated.
