template for pain diary for children?

long shot here.
dc has a long term painful condition that is (we think) improving.
but to measure it we would like to have something simple to record it. are there templates for such a thing?
like a painscale, when it's better, when worse?

I think Great Ormond Street Hospital had a version of this with different activities/times of day but I don't know if it is accessible from their website. It's 6 years since I was there with ds so don't think I've got a copy now. I seem to remember something like this was part of their assessment tool when ds went for appointments.

You could make your own up using the faces scale. Is that the sort of thing you mean?
