Uti, refusing antibiotics

Dd was prescribed antibiotics for a uti about 2 weeks ago. She would not take them, we tried everything including pinning her down and forcing them in - she just forced it back out. Spoke to dr, definitely no alternative medicine so we could try and flush it out. This seemed to be working and until today she had stopped wetting herself and complaining of sore bits. Unfortunately today we are back at square 1 so the natural option clearly hasn't worked. We have had a stand off (she now has nothing in her bedroom apart from a bed as it has all been removed) and the dose we managed to force into her has been spat all over the sofa. I want to cry.
We have tried to reason with her (she is 4) and explain that we love her and want her to get better and the only way to do that is by taking the medicine. She seems to get this but still won't take the medicine.
What can we do? Could she do herself some real damage if it doesn't get cleared up?

Ds1 used to be like this, we tried bribery, begging and wrapping him in a towel and forcing it in, he would choke though or try and spit the medicine out. In the end we did it 1 ml at a time and held his mouth shut from under his chin and he did swallow it but it was horrible.
