Cavity on 21 month olds tooth??

Dd becomes hysterical when try to brush her back teeth. I am insistent to the point of pinning her down but it's a struggle and is often sick from the hysteria. She lets me brush the front me fine.

I worry the back ones aren't being brushed properly and today noticed a very small black dot on her back molar.

Is this a cavity? What can I do? Making an app with the dentist for this week.

Feel like a bad mum.
