Newborn constant poo and terrible nappy rash

dd is four weeks old and seems to leak poo all the time, literally every ten minutes or so. She is breastfed and it is normal breastfed baby poo, though watery, and she is otherwise well and mainly content, putting in weight. But she has a horrible raw rash in spite if frequent attempts at nappy off time and using metanium cream. Does anyone have any better treatment ideas? Should I take her to the docs?

Could the rash be thrush?

If she's pooing constantly, maybe it's a reaction to something - cow's milk maybe?
