Just teething or something else? Slapped cheek?

15 month old has been cutting first molars over the last fortnight and has been a little bundle of misery (just generally crying and in pain). Only one left to cut now. Tonight when I put him to bed he was really shivery and had a temp of 39.3. Also weird rash on cheeks - like a pale circle surrounded by a bright red, very hot patch - only high up on cheekbone, not main bit of cheek. I would have put the rash down to teething, but it seems weird that he has it both sides when he's only got one molar coming through. Also, temp seems much too high for teething. Makes me think it's something else - maybe slapped cheek - but the rash doesn't look like any of the pictures I've looked at on google. Any thoughts please?

Ear infection? High temp + red cheeks often symptoms.

Ds had slapped cheek, it was very distinctive - his entire face apart from forehead and chin were BRIGHT BRIGHT red like sunburn and the rash then spread everywhere esp his arms and legs - red and sore of patchy/lacy.

Temp is too high for teething I think.

I'd lay money on it being an ear infection tbh.
