A perforated eardrum would. If he is young (less than 3/4) he may have had an ear infection without you realising. Maybe ask your doctor to check.

Has he always reacted like this? If not, check it out.
My DD always kicks off massively when water goes in her ear - there's nothing wrong, she just hates the sensation of it.

hes 5,hes not had temp or pain just getting over a cold though?had perforated ear end of last year.could it possibly not have healed or something?
hes always loved water,doesnt mind his hair bein washed

DS had a perforated eardrum about a month ago without any symptoms/signs of his usual ear infections. I would take him to the docs for a quick check in case there is any infection
