Book for recording details of new baby for siblings to fill in

I saw it on amazon and it got good reviews, but I can't seem to find it again. Basically the siblings fill in details such as eye colour etc etc and details about themselves too, like their favourite colour. I thought it would be good for my nephew who is throwing lots of tantrums at the mo, probably caused by attention younger one is getting but possibly because mum is pregnant with another.

I have a book similar to this that I bought for DD1 when I was pg. In fact she was too young for it (2.5). It is stashed on a shelf in the DDs bedroom. Don't dare go up there now and start crashing around, but if no-one else can help, I'll be back tomorrow.

We are trying for baby number 3 at the moment. If it happens I would def consider it for DD (she is 5)
