CMs - how do you do your permission forms?

At the moment I am making a permission form for each time I go to somewhere new and then put a line in at the end that makes it a blanket form for taking them there again whenever I feel it's appropriate. Is this what you do, or do you just have a policy that says you can take them wherever you fancy, whenever you fancy - is this appropriate for the ages that I have - I currently have one 16m/o and some school age children.

My paperwork is quite frankly woeful and I really need to pull my socks up, so any input on what policies you have as well as what permission forms you get signed would be massively helpful - thank you MN ladies (and men!)!

Mine is basically any outing, anywhere, any time.

I do not use NCMA contracts, preferring instead to write my own so I include all the "permissions" (outings, going in my car etc.) in the contract. Inspector said that was absolutely fine.

I just have blanket 'outing' forms too. Too much trouble otherwise! Here's what mine say:

Outings Parental Permission Form

I give permission for my child to be taken on routine outings to the children?s centres, shops, schools and pre-schools and local facilities without being asked each time.

I also give permission for my child to be taken on special trips, for example to a museum, however I would like to be advised of all such trips in advance where possible.

Then spaces for all of us to sign at the bottom! Hope that helps!

I just have one to give permission to leave the premises and another to go in my car, think that covers all eventualities!
