Profiles - Early learning goals sheet

I am just starting back after maternity leave and need to make up a new profile folder.

In the folders I had I was putting in a sheet which had the six areas of learning in boxes with the milestones in and they were divided into the various age categories eg 22-36 months. I was then ticking off and dating when they met the goal eg are able to stop

Does anyone know where I got them from???

They are in the EYFS practice guidance.
