Parent feedback forms for childminders

I've heard it's good to get parents to fill these in, kind of like a reference, but have no idea where to get them from... is it something I have to devise or are there sample forms somewhere?

i just have a form that asks parents to comment on the service i provide, anything they think i should improve etc.

i had my first ofsted last week and the inspector said they were fine.
