hiya,

what information forms do you use with your parents for your mindees? im scma registerded and we have small a5 form whch dont really cover a lot of information, do you use ready made ones or anyone made there own?

Do you mean a registration form of some kind, with details of the parents and children, such as address, phone numbers, emergency contact name & phone number, children's dob's, childrens allergies etc?

I use the ncma child record forms, they have all of that on them. Guess you could do your own easily enough though

I'm not a childminder, but I use a very basic form for babysitting.

Hunting around Google, found an example of a Child Record Form (page 110 of Childminding: A Guide to Good Practice) should anyone be wanting an example of a record form.

I use my own:

  1. Child Info - including gp & health visior details & immunisations, languages spoken at home, religion practiced etc
2 Parents details - for each parent:- name & home address, place of work address, work/mobile/home telephone. Who has parental responsibility, does anyone else have a legal right to access? who should first be contacted in an emergency?

3 List of authorised people to collect the child

  1. Permission for outings/day to day trips
  2. Permission to seek emergency treatment
  3. Permission to take photos
  4. Permission to do observations
  5. Animals on Premises (details of any known animal allergies or aversions, list of the animals I look after when owners away)
  6. Face Painting permission (confirmation no allergy)
10. Permission for occasional treats (whether child allowed ANY sweets, chocolate, icecream, biscuits or cake) 11. Administering medicines (permission to use plasters, teething gels, suncream etc as well as if any inhaler, epipen or anything else used regularly) 12. Child protection (all parents to sign that they are aware I have a legal obligation to report any concerns without necessarily advising them I will do so etc)
