Questions to ask childminder?

I am going to see a childminder this evening, my son is 7 so will be for before and after school

What sort of questions should I be asking please?

At the moment I have "How much do you charge?"

In school holidays its usually a mix of staying with my parents, MIL or we take holiday from work so could ask if she does holidays too

Apart from that anything else I should be asking?

How many children does she have in any one session?

Does she provide food or do you need to send it in with him?

Any holiday flexibility?

When does she take her own holidays?

How many other kids/what ages
What activities will they do
Will he get food and drinks or provide his own?
What is the sickness policy for both the children and the childminder
Are any other adults or children living in the house /be present when he is there, are they DBS checked also
What happens if you are unavoidably late for pick up
