Can anyone please tell me whether you have to provide proof that you have earned the minimum amount to qualify for the free 30 hours childcare when you apply for the code please? DH earns enough but I won't earn enough until DS actually starts preschool as then I can pick up extra shifts at work. Do you have to already be earning the minimum to apply?

You can only apply for Tax-Free Childcare and 30 hours free childcare, if you’re starting or re-starting work within the next 31 days.
You should try to apply 5-7 weeks before the eligible term starts, so for a January start you really want to apply by end of November but that may not be possible given you are not currently in work and have the max 31 days before re-starting work.

You need to expect to earn, on average, the equivalent of at least 16 hours a week at the National Minimum Wage or Living Wage, for the next 3 months. So you can work less than 16 hours and still be eligible as long as the salary you get is at least equivalent of working 16 hours at NMW.
