Hi, has anyone out there applied for working tax credits/ child tax credits via Universal Credit? If yes, do you need to provide any supporting documents? If yes which ones?

Once you apply for UC, you'll no longer get tax credits as UC is replacing tax credits. When I applied for UC, I had to go to the JobCentre and provide my DS's birth certificate, my proof of my ID and home address.
