Need some help

Long story but some years ago I was a childminder and I have a friend bringing some children over to the UK (I live in Italy) for a full immersion English course. I mentioned she needed to get some permission slips from the parents just in case. Wondered if anyone had a recommended wording they could advise. I think she will need something to say in case of emergency they have given heir permission for any necessary medical treatment.

Any advice gratefully received standard form even better if anyone has one.

something along the lines of..

I, ...(parent name) agree that ..(your friends name) is authorised to obtain medical treatment if nessecary whilst my child ...(thier name) is in her care on the ..(name of trip).

My child is/is not allergic to ..........(any allergies medical or otherwise)
My child doctor in italy is ..(docs full name, practice addres and contact number)
I understand ...(friends name) will firstly try to contact myself or any one of our other contacts prior to recieving any treatment, but agree that if no contact available she should do whatever nessecary to help my child.
