My network co-ordinator has recently given me a simple risk assessment form (for childminders) - I just need to record the date of assessment, and to put in what the hazard/s is/are, where it is, who might be harmed and how, and what can be done to minimise the risk.

A second sheet gives more detail on how to fill in the form. It states that a hazard is a danger or something that could cause harm to somebody. A risk is the chance of a hazard actually causing harm.
