How do you write an invoice?

I know this is probably a really basic question but I need to write an invoice for my first parent, who starts on Monday.
She is paying me monthly in advance and I said I would do her an invoice.
I have never written one before and don't have a clue how to lay it out.
Can anybody help?

Look on www.microsoft.com - they have templates you can download into Word.

I just use a duplicate book (from all good stationers!!) for each family - I just write the child's name, the hours/days and how much dosh it amounts to. I obviously give the top copy to parents, and write "paid" on my copy (assuming that they do!) It's really very technical! Smile
