Today I've had to fill this in for the first time and I'm a little confused. I've got the NCMA book for recording this and it states that a carbon copy should be given to parents yet each page has the section 3 times (i.e. name of child, injury etc.) which suggests the next time I use the book, I use the next bit down on the same page. How can I therefore give a copy to the parent and if I do, then the other 2 sections are a complete waste and I can't see why they would be there?????

So, do I or do I not have to give a copy? (it's been signed by parent)

Isnt it like the medicine bit and you fill it in from the bottom?

I've read the instructions at the front of the book and nowhere does it say do from bottom to top on anything. It doesn't even mention giving a carbon copy for medicine, just says to sign which is what I've always done. I would have thought they'd say give carbon copy if this was required for consistancy throughout their own instructions iyswim.

The bottom to top idea is a good way round it yes, but it doesn't say it in the book. What do the rest of you do?

I do it from the bottom upwards but it caught me out the first time I did it too

Oh and yes you have to give the parent a copy.

Cheers, will do that. When you say you have to give parent a copy, do you mean medicine aswell? (it doesn't say to in the book and there are no carbon copies for long term medication forms).

I know I have to for the other thing as it says in the book.

Ok not the continuous medication but the rest all seem to have carbon copies so i guess everything that has carbon copies

Suppose it makes sense it's just that the book says carbon copies for some things but not that section :( I'm easily confused Wink

Will give copies of what I have so far (it's only 1 mindee) and will do from bottom to top from now on! :)

Helly i work from the bottom and it caught me out so i wrecked the first page, the first time. I give a copy of the medicine bit so the parents can refer to that evening before giving medicine becuase that is what i would do before i would give meds.

Right then, I'm not going mad - thought you were saying it was in the book that you went bottom to top and I was just blind Grin. I will give from now on to cover myself although I've always recorded all medication in their individual daily diary's so they would know the time etc from that.
