Hi all, have my final date to be registered on 26th April with Care commission and i'm in a muddle trying to do all my forms, i had an operation 4 weeks ago(self inflicted) and the recovery has taken longer than expected therfore i haven't got round to typing up the required forms, i have done my parent hanbook, all my policies and procedures but i still need Forms for: Childrens Details/emergency contact, Parental Consent form for administration of medication, Accident/Incident/Restraint form,
A Fire Drill Record sheet,
Permission forms for outing and suncream/plasters to be used, would any of you be able to type up these forms for me and send them to me, i'll pay all costs obviously, if anyone can help could you email me, traceydotloganattiscalidotcodotuk thanks

(a complete aside here kidstrack2..... are you selling any 18-24m girls clothes at all? DT1 is only just beginning to outgrow the 12-18m clothes you sold me before. Everytime she wears them I think of you. The clothes were gorgeous... so I thought I'd see if there were more where they came from. Mars)

Are you going to be a ncma member if so some of the forms are in the packs to save you doing it.

I just sent you an email about the consent forms. Ill send you a copy of the scma ones if you want.

Did you do the SCMA cousre. I did this last year and got a voucher for a years free membership, insurance and cashbook/register, contracts and forms for when I registered. I made it clear in my policies that I would use these forms and the CareCommission were happy with this.
I've done my own photo consent form that I can email you if you like

allie no didn't do that course it was one run by Stepping Stones in South Ayrshire, i have down loaded a lot of info without joining SCMA as i don't feel they would be any great benefit to me, i have put my insurance through Morton Michelle. So far i have done all paper work myself and the Care commission were impressed with all that i have done, just a bit behind as i had an operation, i would have to travel to do the SCMA course, sounds as though you got a good deal by doing their course! Thanks to all that have helped, really appreaciate it!
