Which do you have and how do you get parents to sign?

I'm updating all my paperwork and wanted to see what other CMs request permission for. Also, I'm considering condensing it all to one sheet and getting parents to sign just one form to say they agree to all permission requests.

Hi,

I'm new to childminding so have only recently put mine together, I have permission requests for:

Photographs
Sun cream
Nappy Cream
Large Play equipment (such as trampolines / park equipment)
I'm sure there's one more but I can't think of it off the top of my head.

The oftsed inspector at my registration visit said that I need a generic one for outings. By this she didn't meanspecial trips out but just everyday ones such as to local playgroups / library / school run etc.

I'll be interested to see what other CM's have in case I've missed any!

Oh and I have them all on one sheet - musch easier and more environmentally friendly!

what about one for sleeping in the buggy!hand gel,emergency treatment,public transport,swimming,sorry tahts all i can think at the moment

Hair combing
Sun cream
Tooth brushing
Outings
Emergency treatment

Haircombing? Really?

Yes ... because I might want to comb 'visitors' out of their hair :)

Washing bedding/towels (ie OFSTED wanted them washed daily; as I use individual I get a signature to say they are washed weekly)
