Planning an activity, advice please

An assessor is coming to see me on Wednesday. Need to plan an activity to carry out while she is present.

What information do I need to include. Theme All About Me. I know what I want to do just need to know how to get it down on paper.

I think I would do it a bit like a recipe for cooking - what you need ... what to do ... and then you might add something about extensions, EYFS links, etc? Maybe? You could include something about the ages/abilities of the children? Maybe even why you're doing it? What do I know ...........
