Both my babies, more than 8 lb with larger thn averge heads(ds1 has to wear 6-8 yr old hats - he is 3), left me with no tear at all. I am only 5'2 so did not expect this. Am also not particularly earshaped.

Well I've noticed in the x-ray dept they have different sized gonad shields. Must be very embarrassing for the guy thats handed the small size.

i had secondary tears & lots of stiches with ds1 who was 5lb14oz and just small tears/grazes with ds2 who was 5lb4oz, both were inductions so maybe that adds to the tearing?
